Most people improve <strong>without surgery<\/strong>, with exercise, pain medication and, when needed, injections or radiofrequency ablation. Surgery is reserved for nerve compression that does not respond to conservative care.<\/p><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">What are the facet joints?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Each vertebra connects to the next through a disc in front and two small facet joints at the back. The facets guide and limit movement, especially twisting and bending backward. Like the knee or hip, they are lined with cartilage and surrounded by a capsule, so they can develop arthritis.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What is facet hypertrophy?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">As the cartilage wears down, the joint reacts with bone spurs, a thicker capsule and inflammation. On an MRI or CT report this is often described as &ldquo;facet hypertrophy&rdquo; or &ldquo;facet arthropathy&rdquo;. Interestingly, a 2018 MRI study of 200 people found that the facet joints of patients with spinal stenosis were not larger than normal but narrower, and its authors argue the name is a misnomer. Imaging alone is not enough, because facet changes are very common in people without pain. When it is unclear whether the facets are the source, doctors use a <strong>diagnostic medial branch block<\/strong>: a small dose of anesthetic on the nerves that supply the joint. The randomized trial among them (46 patients) suggested that steroids gave faster relief but PRP lasted longer at 6 months, and a later study compared PRP with corticosteroids for facet synovitis. The authors agree that larger, better-designed trials are needed.
