None of the large provincial plans we reviewed (OHIP, MSP, AHCIP, RAMQ) pays for <strong>elective, experimental treatment abroad<\/strong>, so expect to pay out of pocket and to buy private travel insurance. The CRA <strong>may<\/strong> accept amounts paid to a licensed practitioner or licensed hospital outside Canada for the Medical Expense Tax Credit, but whether a specific claim qualifies is decided by the CRA, not by the clinic.<\/p><\/div>\n\n\n\n<p class=\"wp-block-paragraph\">Canadians who look into stem cell therapy for knee, hip or shoulder pain quickly run into three questions: is it allowed, will my province pay for it, and can I deduct anything at tax time? This guide answers each one using the official pages of Health Canada, the provincial health plans, the Canada Revenue Agency and the Government of Canada travel advice. It is general information, not legal, tax or medical advice, and the rules can change, so check the linked sources before you decide.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What Health Canada says about stem cell therapy<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">In a public advisory issued on May 15, 2019, Health Canada warned that some clinics and practitioners were offering unauthorized cell therapies, including stem cell therapy, while suggesting they were safe and making unproven health claims. The key points of that advisory are:<\/p>\n\n\n\n<ul style=\"list-style:disc outside;padding-left:1.3em\"><li><strong>Cell therapies are drugs.<\/strong> All cell therapies are considered drugs under the Food and Drugs Act, so they must be authorized by Health Canada before they can be offered to Canadians.<\/li><li><strong>Very few were authorized.<\/strong> At the time of the advisory, the only authorized products were one stem cell therapy for graft-versus-host disease and two cell-based gene therapies for certain cancers. None was for joint or back pain.<\/li><li><strong>Most are experimental.<\/strong> Outside those products, cell therapies may only be given in Canada within a clinical trial authorized by Health Canada.<\/li><li><strong>Names vary.<\/strong> The advisory names &ldquo;BMAC&rdquo;, &ldquo;SVF&rdquo; and &ldquo;adipose-derived stem cells&rdquo; as examples of autologous therapies offered by for-profit clinics.<\/li><\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Health Canada has also been studying how to regulate a patient&#8217;s own cells prepared at the point of care for orthopedic use. It held a workshop on the subject in March 2021 and said it would consider a new pathway for advanced therapeutic products. In other words, the Canadian framework is still evolving, and so is the scientific evidence for these treatments.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">One thing the advisory does not do is regulate care delivered in other countries: it is about what can be offered in Canada. Experimental or developmental treatment is not eligible.<\/td><td style=\"border:1px solid #e3dcd8;padding:10px 12px;text-align:left;vertical-align:top\">Maximum CAD 75 per day for hospital care.<\/td><\/tr><tr><td style=\"border:1px solid #e3dcd8;padding:10px 12px;text-align:left;vertical-align:top\">Alberta (AHCIP)<\/td><td style=\"border:1px solid #e3dcd8;padding:10px 12px;text-align:left;vertical-align:top\">Elective (non-emergency) treatment and surgery outside Canada is listed as not covered.<\/td><td style=\"border:1px solid #e3dcd8;padding:10px 12px;text-align:left;vertical-align:top\">CAD 100 per inpatient day; CAD 50 per outpatient day.<\/td><\/tr><tr><td style=\"border:1px solid #e3dcd8;padding:10px 12px;text-align:left;vertical-align:top\">Qu\u00e9bec (RAMQ)<\/td><td style=\"border:1px solid #e3dcd8;padding:10px 12px;text-align:left;vertical-align:top\">Professional services are reimbursed only if the same services are covered in Qu\u00e9bec, and never above Qu\u00e9bec rates.<\/td><td style=\"border:1px solid #e3dcd8;padding:10px 12px;text-align:left;vertical-align:top\">Hospital services after a sudden illness or accident: up to CAD 100 per day, CAD 50 per outpatient day.<\/td><\/tr><\/tbody><\/table><\/div>\n\n\n\n<p class=\"wp-block-paragraph\">Because stem cell therapy for joint pain is not an authorized treatment in Canada, it is very unlikely to meet any of these criteria. Plan on paying for the procedure yourself. All four provinces also recommend private travel insurance, and the Government of Canada notes that <strong>most travel insurance policies do not cover planned medical procedures<\/strong> in another country, so ask your insurer in writing what is and is not covered, including complications.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Health Spending Accounts and private plans<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Many Canadian employers offer a Health Spending Account, which the CRA calls a health care spending account and treats as a private health services plan. For the plan to keep its tax status, at least 90% of what it pays must be for expenses that qualify for the Medical Expense Tax Credit. Whether your account will reimburse a procedure abroad depends on the plan&#8217;s own terms and on the administrator&#8217;s reading of the CRA rules, so ask before you travel and keep every invoice. Anything an HSA or insurer reimburses cannot be claimed again on your tax return.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The Medical Expense Tax Credit for care outside Canada<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The CRA&#8217;s general rule is that eligible medical expenses are <strong>not restricted to those paid in Canada or for services provided in Canada<\/strong>. Its guidance says that if you travel outside Canada to get medical services, you can claim amounts paid to a medical practitioner and a public or licensed private hospital. The details matter:<\/p>\n\n\n\n<ul style=\"list-style:disc outside;padding-left:1.3em\"><li><strong>The practitioner<\/strong> must be authorized to practise under the laws of the place where the service is given. In Colombia, physicians are listed in the national registry ReTHUS.<\/li><li><strong>A licensed private hospital abroad<\/strong> is one licensed under the laws of the place where it operates. The CRA says you should keep full particulars of that licence in case it asks for evidence.<\/li><li><strong>Travel costs<\/strong> (transportation and, at 80 km or more one way, accommodation and meals) are claimable only if substantially equivalent services were not available near your home, the route was reasonably direct and it was reasonable to travel there.<\/li><li><strong>Purely cosmetic procedures<\/strong> are not eligible.<\/li><li><strong>The threshold:<\/strong> you can only claim expenses above the lesser of 3% of your net income or a fixed amount the CRA updates every year (see the current figure on the CRA site).<\/li><\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">We cannot tell you whether the CRA will accept a claim for a stem cell procedure or the travel to get it: that depends on your circumstances and on how the CRA applies its rules. Keep invoices, the physician&#8217;s registration and the facility&#8217;s licence, and talk to a tax professional before you count on a credit.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">A realistic checklist for Canadian patients<\/h2>\n\n\n\n<ol style=\"list-style:decimal outside;padding-left:1.3em\"><li><strong>Talk to your Canadian doctor first<\/strong> about diagnosis, conventional options and whether air travel is safe for you after a procedure.<\/li><li><strong>Verify the clinic and the physician<\/strong> in the Colombian registries (REPS and ReTHUS) and ask where the cells come from.<\/li><li><strong>Get a written quote and treatment plan<\/strong>, including follow-up after you fly home.<\/li><li><strong>Ask your insurer and HSA administrator in writing<\/strong> what they will and will not cover.<\/li><li><strong>Keep every document<\/strong> for the CRA: invoices, licences and medical records.<\/li><li><strong>Read the Government of Canada travel advice<\/strong> for Colombia, which warns that Canadians have died or had serious complications after cosmetic or other elective surgeries there. Without prior approval, you pay all costs.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Can I claim stem cell therapy abroad on my Canadian taxes?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Possibly. The CRA allows amounts paid to an authorized medical practitioner or licensed hospital outside Canada, and travel costs only under specific conditions.
